TRAUMATIC FACIAL INJURIES FOLLOWING ROAD TRAFFIC ACCIDENT

A male patient of 48 year old presented with severe facial trauma after falling from an auto rickshaw. The patient was reportedly intoxicated at the time of the incident, which contributed to the loss of balance and subsequent fall. This led to significant facial injuries, resulted in inability to open the eyes, deformity and swelling of the nasal area, with restricted mouth opening. On CT examination patient had fractures of the nasal bone, eye walls (orbital fractures), and the lower jaw (mandibular fractures).
SURGICAL INTERVENTION :

The aim was to restore both function and cosmetic appearance.
Tracheostomy was performed to secure the airway and facilitate general anesthesia for subsequent surgical intervention.
Miniplates and screws were used to fix the fractures.
POSTOPERATIVE CARE: 
The patient was closely monitored for potential complications, including infection, bleeding, or airway issues. Pain management, wound care, and nutritional support were also provided during the recovery phase.

OUTCOME:
After 3 weeks of hospitalization, the patient was discharged without complications. 
Follow-up visits confirmed that:
• The patient has mouth opening of 3 finger breadth with normal TMJ movements. Also patient regained his bilateral eye movements with vision being intact
• There were no signs of infection or other complications post-surgery
CONCLUSION:

This case highlights the severe impact that even seemingly minor falls can have when combined with alcohol intoxication. 
It emphasizes the need for quick and effective medical care following facial trauma, as these injuries can severely affect an individual’s ability to perform basic functions like eating, speaking, and even breathing.
 
This case serves as an important lesson for the public on the risks associated with alcohol consumption and the potential for accidents, even in familiar or low-risk situations.
 It is essential to prioritize safety—whether using protective gear like helmets, maintaining caution in public spaces, or avoiding risky behaviors when intoxicated.
